PASSPORTS

PASSPORT BOOK is REQUIRED for FLYING

PASSPORT CARDS are for TRAVELING by LAND or SEA to MEXICO, CANADA, BERMUDA and the CARIBBEAN

 Passport Applications may be picked up at the Clerk’s Office or you can go online at www.travel.state.gov

 Applications Must Include:

  1. Form DS-11
  2. Provide Evidence of U.S. Citizenship

You must submit one of the following. Your evidence must be an original or certified copy.  Photocopies and notarized copies are not acceptable.

  • Fully-valid, undamaged U.S. passport (may be expired)
  • U.S. birth certificate that meets the following requirements:
    • Issued by the city, county, or state of birth
    • Lists your full name, date of birth, and place of birth
    • Lists your parent(s)’ full names
    • Has the date filed with registrar’s office (must be within one (1) year of birth)
    • Has the registrar’s signature
    • Has the raised seal of the issuing authority
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization
  • Certificate of Citizenship

  

  1. Bring a Photocopy of U.S. Citizenship Evidence Submit a photocopy of the front and (back, if there is printed information) of the U.S. citizenship evidence you are submitting.

Photocopies must be:

  • Legible
  • On white 8.5” x 11” standard paper
  • Black and White
  • Single-sided

If you don’t want to submit a photocopy of your citizenship evidence, you may submit a second certified copy of your citizenship evidence, which we will submit with your application.

  1. Present Valid Photo ID

You must present one of the following identification documents (ID’s).  The ID must readily identify you.

  • Fully-valid, undamaged U.S. passport (may be expired)
  • Fully –valid U.S. driver’s license
  • S. military ID or military dependent ID

Please note:  If you present an out-of-state ID, you must present an additional ID.

  1. Bring a Photocopy of ID

Submit a photocopy of the front and back of each ID that you present when you apply.

Requirements:

  • Photocopy must be on white, 8 ½” x 11” standard paper
  • Photocopy cannot be double-sided
  • Do not decrease the image size (although you may enlarge it)
  1. Provide a 2” x 2” Photo

  1. Payment of Fees:

The Application Fee and the Execution Fee Must Be Paid Separately

The Application Fee must be paid with a Check or Money Order payable to the U.S. Department of State.

The Execution Fee must be paid with Cash or Money Order payable to DeKalb County Clerk’s Office.

***Ages 16 or 17 are required to have a parent/legal guardian present photo identification if the minor does not have identification of his or her own.  A Learner’s Permit is NOT valid identification.
